51²è¹Ý has partnered with Catapult Health to provide you with a health checkup at no cost to you, as part of your health care benefits.

There are two options to participate this year.
We will have 51²è¹Ý onsite appointments in Hughes-Trigg and in Expressway Tower, and a Home Kit.

*all onsite appointment are full, but you can still order a home kit through May 12, 2023

Option 1 | 51²è¹Ý Onsite Appointment

 

To begin the process, please go to to schedule and confirm your appointment onsite at 51²è¹Ý.

 

  • Schedule your onsite appointment at Hughes Trigg or Expressway Tower
  • Get a quick blood draw and blood pressure check
  • Schedule your virtual Consultation
  • Meet virtually with your nurse practitioner to get a complete review of your results, a personal action plan, and learn about fully-covered wellness programs that may benefit you

 

Onsite appointments will be held at the following locations and dates:

  • March 3, 2023, and March 9, 2023 – Expressway Tower
  • March 6, 2023, March 8, 2023, and March 10, 2023 – Hughes-Trigg Ballroom C

Option 2 | Home Kit

 

To begin the process, request a Home Kit at

 

  • Collect your sample at home in just 10 minutes
  • Return your sample in the pre-paid UPS overnight envelope, then watch for an invitation to schedule your consultation
  • Live video consult with a nurse practitioner, on your device, at a time chosen by you
  • Review your results, then create a personal action plan with your nurse practitioner while learning about fully-covered wellness programs that may benefit you

 

Virtual kits can be requested now through May 12, 2023.
